Event details form

Please fill this form out and tell us about your event. Upon completion and submission of this form, you will receive an Event Contract to sign that has all of the event information that you shared. This is confidential information and will not be shared with anyone else outside of our organization. If DJLu has a conflict with your event date, you will receive notification of such. To secure the date, DJLu must receive a signed contract, and at least 1/2 of total as a deposit. Receiving deposit shall constitute a signed contract.
